The appointment request form on this Website is provided through Microsoft 365 Forms under a HIPAA Business Associate Agreement. Submitting the form is a request only; it does not guarantee an appointment or reserve a time, and no appointment is confirmed until our staff contacts you to schedule it, verify insurance, and finalize your booking. If you provide your telephone number, appointment reminders and text messages are sent only where you have given consent (verbal or written), and you may opt out at any time. Please do not include unnecessary sensitive information in the form, and if your need is urgent, call us at (415) 857-1151.
