Documentation services — evaluations, letters, and disability paperwork — are administrative services that are not covered by medical insurance and require payment at the time of request.
Good Faith Estimate: If you are uninsured or paying out of pocket, you have the right to a Good Faith Estimate of expected charges under the No Surprises Act. Please contact our office to request one.
